A note on Adobe Acrobat Reader

Some of our forms are available in pdf format.  To access these forms, you must have Adobe Acrobat Reader.  Acrobat Reader does not allow you to save information that you typed into the forms.  Before you fill out any forms using Acrobat Reader, make sure that you have all necessary information at hand and access to a printer.

Application for Teaching Assistantship (TA) 

Who must file: Any Graduate student who wishes to be a TA. 

Filing Instructions: This form is available in writable pdf format.  To access this form, your web browser must have Adobe Acrobat Reader.  Complete the form, print it, sign it, and return it to the department. 

Application to Candidacy (ATC) Form and Instructions 

Who must file: All students applying for their M.S., M.Eng., or Ph.D. degree conferral. See instructions and deadline timetable included. 

Filing Instructions: This form is available in Microsoft Word format. Complete the form, print it, sign it, and return it to the department.  Please note that often you need to complete an Independent Study / Variable Credits Hour Course Description Form and/or a Certificate of Full-time Status form to accompany the application. 

Certification of Full-time Status 

Who must file: Students who require full time status.

Example of MS, MEng, or Ph.D. Defense Announcement 

Who must file: All students defending their master's project, master's thesis, or doctoral dissertation. 

Filing Instructions: This form is available in pdf format.  To view this form, your web browser must have Adobe Acrobat Reader.  Stationary available in the Graduate Secretarial Office. Defense announcement must be posted 2 weeks prior to defense date. 

Graduate School Multi-Purpose Report (M Form) 

Who must file: All graduate students defending their project, thesis, or dissertation for conferral of their degrees. The M-Form must be available at their masters and doctoral final defenses for signatures of the major advisor and committee members. 

Filing Instructions: The M-Form is prepared by the graduate secretary and given to the major advisor on or before the defense date. The student is responsible for notifying the graduate secretary prior to the defense and making sure that the major advisor receives the form. 

Graduate Student Petition Form 

Reason to file:

Leave of Absence request 

Extension of time limit for completion of degree 

Extension of time limit for change of "I" grade 

Undergraduate course for Graduate credit 

Waiver of Continuous Registration Requirement 

Amendment to Application to Candidacy

Individual Problems / Variable Credit Hour Course Description Form 

Who must file: Any graduate student taking independent study / individual problems or special topics. 

Filing Instructions: This form is available in writable pdf format and writable Microsoft Word format.  To access the pdf form, your web browser must have Adobe Acrobat Reader.  Complete the form, print it, sign it, and return it to the department. 

Master of Science Thesis Form 

Who must file: All M.S. students in good standing must complete this form, signed by the student and by the Thesis supervisor. The deadline for filing this form with the Graduate Secretary is the Friday of the sixth week in student's second semester in the Master of Science program.

Ph.D. Outside Reader Appointment Form 

Who must file: The Outside Reader Appointment Form must be submitted to the Department for approval when the outside reader has been identified. The dissertation should NOT be given to the outside reader before he/she has been approved by the Department.

Ph.D. Outside Reader Response Form 

Who must file: All doctoral students must forward their dissertation to an outside reader with this form to be completed by the outside reader.

Ph.D. Publication / Presentation Requirement Form

Who must file: All doctoral students must fill this form at the time of the dissertation defense. Without such a letter, the Director of Graduate Studies will not approve the M-Form certifying satisfactory completion of the program requirements. It is the responsibility of the student to make sure that this important and final requirement is met before the approval of the M-form.

Ph.D. Qualifying Exam / MS Comprehensive Exam Registration Form

Who must file: Students must take and pass this examination within four (4) semesters of admission to the graduate program.

Those who enter the Ph.D. program after completing an M.S. degree in EE at UB (other than the All Course Option), must take the examination within the next two (2) semesters immediately following the M.S.

M.S. students choosing the All Course Option must pass the examination within four (4) semesters of admission.

Request for Electing or Changing EE Faculty Advisor 

Who must file: All graduate students must select an advisor when they enter their degree program. This form can be used to select or change the faculty advisor.

Request for Transfer Credit 

Who must file: Any graduate student requesting transfer credits.

Student Address Form 

Who must file: All students must notify the department of their current address or any address change.

Tuition Scholarship Verification Form and Instructions 

Who must file: Students who have a graduate, teaching, or research assistantship.

Request to Amend Tuition Scholarship Verification Form 

Who must file: Students who have a graduate, teaching, or research assistantship and are making a change to their original tuition scholarship form.

Filing Instructions: This form is available in pdf format.  To access this form, your web browser must have Adobe Acrobat Reader.  Print the form, complete it, sign it, and return it to the department. 

TAP Excess Income Affidavit Form 

Who must file: Students who have a graduate, teaching, or research assistantship and not eligible for TAP because of excess income earned.
